Current Ministry Update Form
Whether you're the leader of a new, returning, or on-going ministry/Bible study/small group, please take a moment to fill out this form to provide and ensure the current information about the ministry you lead (topics/resources used) can be reviewed by Pastors/Elders, requests for facility/room usage can be made, and any changes to or cancellation of the ministry are received.
New Ministry Request Form
This form is to be filled out completely and with as much detail as possible about the ministry you are looking to create prior to an interview with the Director of Ministries. The person filling out this form, must be the leader of the ministry and a member of Goodwill Church. The group mission must be in line with Goodwill Church's vision and mission.
Special Event/Room Request Form
This form is to be filled out when your ministry is planning to host a special event or needs to reserve a space to use at Goodwill Church. The person filling out this form must be the leader of the ministry. The event must be in line with Goodwill Church's vision and mission. All requests will be reviewed and approved by operations staff.
